Welcome to the U.S. International Health Alliance elective in El Petén, Guatemala. Below I have listed a number of things we will need from you to facilitate your participation in the elective.

Forms and Insurance
In anticipation of your arrival we need copies of the forms listed below ONE MONTH PRIOR to your arrival in Guatemala.

Please send the completed and signed forms to the following address: U.S. IHA, 61535 S. Hwy 97, Suite 9-305, Bend, OR 97702.

  1. Application
  2. References
  3. Assumption of Risk
  4. Health Disclosure
  5. Emergency Contact Information
  6. Code of Conduct
  7. Media Release

We will also need a copy of the front and back of your Health and Evacuation Insurance card. Information about getting international travel insurance can be obtained at the Center for Disease Control website or Sunburst Worldwide Insurance Services. Be sure to bring the copies of these documents with you to El Petén.

Travel Information
We anticipate that the students will ARRIVE AND DEPART TOGETHER. Ideally, the students should plan on arriving Sunday one day prior to the elective and departing on the last day – Friday – of the elective – or the Saturday following the last day.

Along with the documents, we will need the dates of the student’s arrival and departure and the airline information so that pickup at the airport can be arranged.

Reservations should be made to Guatemala City and then to Flores. TACA has 2-3 flights daily from Guatemala City to Flores – 6:30 am; 9:30 am and 5:00 pm.

Please check the internet to see if there are any changes. (At this time there are no direct flights from the US to Flores.)

Program and Room/Board Fee
Your program fee and room and board fee must be received ONE MONTH PRIOR to your arrival in Guatemala. Please send a check made out to:

Your room and board fee allows you to stay in the mission house beginning Sunday afternoon before your rotation starts and ends Saturday morning the day after you rotation ends. Any other plans for staying in the mission house must be specifically approved and agreed to in writing before hand.
